Hardship Response Intake

We are so sorry that you are currently experiencing hardship, and we pray that it creates steadfast faith within you and that the Lord clings closely to you in this time. The Hardship Response Team exists to faithfully walk alongside of you and your family in your time of suffering this hardship (sickness/medical issue, loss, spousal deployment, or other challenges from living in a fallen world). As fellow covenant members, we would love the chance to serve you and help bear these burdens, but we only want to serve you in ways that would be helpful to you.

If there is anything you need that is not listed, there will be an option at the end to share any additional information with us. There is also a place on the form to alert us of the hardship but let us know that you are not interested in having us intervene.
